malaysia students studies in USA requirements

Hello everyone,
I have questions about US student requirement to get their courses approve by jpa.

1) what is the minimum requirement? etc credit hours, gpa, courses ..etc
2) If i fail a class and retake it, is it gonna be a problem ? ( will not be recognize, stop funding..etc)
