Qisqualification from plkn

Hello all~ i'm disqualified from plkn due to medical condition~ now my question is, what do i do? The letter ask me to send the medical form to plkn n jlkn~ and i dont hav their addresses~ found only jlkn's.. or is it better to go to the pusat to handover the medical form rather than by mail? please tell me what to do!!! *despo* :cry
