Anyone received call that need us to send a medical check up report to JPA?HOW?NO FORM?HOW CAN I DO THE MEDICAL CHECK UP?URGENT

Err, go to the clinic and ask for full medical check up. You will get like a statement stating you've done check up or so. Check with the clinics first.

Hmm, why do you need a med check up huh? Why don't you ask them just now? or call them again?

is there such thing? i haven't heard anything about this medical report matter.... does that mean you've got yourself the jpa scholarhip when they ask you to do a medical check-up?
